Fortunately, no one was injured. On Friday (June 8, 2007), a section of a Master Blaster water slide at Big Splash water park in Tulsa, Oklahoma collapsed with an 11 year old girl on it.

I think the park is remarkably lucky that no one was killed.
Fortunately, this type of mechanical failure is very rare on amusement rides. Most accidents are caused by rider or operator error. The engineer in me desperately wants to know what factors led to this major mechanical failure.
